For this assessment, you are a presenter! You will create a 5-10-minute video using Kaltura or similar software. In the video:
- Propose your evidence-based care plan that you believe will improve the safety and outcomes of the patient in the Vila Health Remote Collaboration on Evidence-Based Care media scenario. Add your thoughts on what more could be done for the client and what more information may have been needed.
- Discuss the ways in which an EBP model and relevant evidence helped you to develop and make decisions about the plan you proposed
- Wrap up your video by identifying the benefits of the remote collaboration in the scenario, as well as discuss strategies you found in the literature or best practices that could help mitigate or overcome one or more of the collaboration challenges you observed in the scenario.

Hello everyone,
My name is xx. This is a presentation on remote collaboration and evidence-based care focused on a 65-year-old patient male patient with congestive heart failure (CHF). Being a rural area resident, the patient’s access to specialized cardiovascular care is limited. He has a history of hypertension and has been having fatigue, shortness of breath, and leg edema. The interprofessional team involved in his care includes Dr. Johnson who is the primary care provider, a cardiologist, a nurse, and a pharmacist.
Evidence-Based Care Plan
Evidence-based practice (EBP), the careful integration of best (current, and available) research evidence, clinical expertise, and patient values and preferences in clinical decision-making and patient care delivery has been consistently shown to improve care quality, patient safety, clinical outcomes, and even variation in healthcare practice (Connor et al., 2023). CHF is among the chronic illnesses that have and continue to benefit from EBP. As described by the Centers for Disease Control and Prevention [CDC] (2024), CHF is a chronic and complex health condition characterized by impaired functioning of the heart. It affects close to 6.7 million 20+ year-old adults in the United States and was listed in 457,212 deaths in 2022, with associated costs in 2012 having amounted to about $30.7 billion (CDC, 2024). Evidence-based interventions, particularly telemonitoring, have been proven to slow CHF progression, improve patients’ quality of life, and clinical outcomes, and prevent deaths (Gupta et al., 2021).
The evidence-based plan for the patient with CHF includes:
- Comprehensive assessment and monitoring
Telemonitoring: Implementing a telemonitoring system to track the patient’s vital signs such as heart rate, blood pressure, weight, and oxygen saturation and help detect early signs of worsening CHF (Masotta et al., 2024). The patient will be scheduled to have weekly virtual check-ins to monitor progress and address concerns.
- Pharmacological Therapy
As recommended by the cardiologist, the patient should be initiated with ACE inhibitors (e.g. lisinopril) and diuretics (e.g. furosemide) to manage symptoms, improve cardiac function, reduce fluid buildup, and reduce morbidity and mortality (Oskouie et al., 2024). Per the pharmacist’s recommendation, the patient and his family should receive education on the importance of medication adherence, potential drug interactions, and side effects (Gupta et al., 2021).
- Lifestyle Modifications
Lifestyle modification is necessary to improve CHF management. The patient plan includes education on avoiding unhealthy lifestyles that cause or worsen CHF, these include smoking tobacco, physical inactivity, and consumption of foods with high salt, cholesterol, and fat contents (Gupta et al., 2021).
Dietary counseling for the patient includes restricting sodium intake to <2g/day to prevent fluid retention, and encouraging a heart-healthy diet rich in vegetables, whole grains, and lean proteins (Heidenreich et al., 2022).
