One clinical client-focused quality measure often analyzed in our clinical practice is patient fall rates. Falls can result in serious injuries and thus, a critical quality measure for patient safety and outcomes (Morris et al., 2022). The measure tracks fall incidences among patients during their hospitalization periods.

The data collected includes the number of patient falls, the circumstances of the fall (e.g., time, location, activity), patient demographics (age, health conditions), and any injuries resulting from the fall. The data is mainly sourced from incidence reports collected and submitted by nurses, and other healthcare staff who document events as they occur. The information is entered into the electronic health record (EHR) system as well as the incident report system. The quality improvement (QI) teams conduct data analysis and involve nurses in reviewing data trends (e.g. high-risk patient groups). QI specialists and nursing leadership report the fall rates to the healthcare stakeholders (administration, staff, patients and families, Joint Commission, and even state health departments.
