Advanced practice nurses apply continuous quality improvement (CQI) processes to improve client-centered outcomes. Select one of the following client-centered care initiatives that you would like to improve in your practice area: client clinical outcomes, client satisfaction, care coordination during care transitions, or specialty consultations for clients.

The selected client-centered care initiative is care coordination during care transitions (CCCT). Effective CCCT (e.g. acute care to rehabilitation care, hospital to home, hospital to nursing home, etc.) ensures delivery of continuous, comprehensive, and patient-centered care, reducing readmission risks, medication errors, and other adverse events (Karam et al., 2021).
Improving CCCT will be crucial in my future practice as an advanced practice nurse (APN). I will lead evidence-based interventions to address fragmentation in healthcare and reduce any confusion that arises during care transitions. My focus will be to improve interdisciplinary/multidisciplinary and provider-patient/ family communication to ensure every patient’s needs during care transitions are met.
The Plan-Do-Study-Act (PDSA) cycle is an iterative, cyclical process that allows for small-scale testing of changes before wider implementation, making it well-suited for improving CCCT (Hill et al., 2020).
The Plan phase would include: defining the problem (e.g. high readmission or medication errors during care transitions), setting objectives (e.g. reducing 30-day readmission rates by 25% over six months, increasing nurse compliance to standardized discharge process by 95% over six months), developing an intervention (e.g. standardized discharge process), and establishing metrics (readmission rates, patient satisfaction scores, and the rate of compliance to follow-up visits)
The Do would involve implementing the plan on a small scale by testing the new discharge process in a single unit before adopting it throughout the hospital (Hill et al., 2020).
The study would involve collecting and analyzing data to determine the intervention’s effectiveness (Hill et al., 2020). For instance, comparing pre- and post-readmission rates, patient satisfaction, and nurse compliance rates.
