The selected performance measure is “Fall Risk Management (FRM)” under the Effectiveness of Care domain. The key components of FRM include discussion of fall risks and management of fall risks (National Committee for Quality Assurance [NCQA], 2024).

FRM is a clinically significant quality and safety issue particularly because falls are a leading morbidity and mortality cause among older patients aged 65+ years, with fall-related death rates having increased significantly by 41% (from 55.3/100,000) in 2012 to 78.0/100,000 in 2021 (Centers for Disease Control and Prevention [CDC], 2024). About 1 in 4 (about 14 million) of 65+ year old adults are victims of falls every year, yet only less than 50% inform their doctors. A history of falls increases one’s risk of another fall by two-fold (CDC, 2024). Current evidence shows the correlation between fall-related injuries (e.g. brain, knee, hip injury) and increased risk of hospitalizations, reduced independence and quality of life, and premature death (Heng et al., 2020; Jewell et al., 2020; Wang et al., 2024). About 1/10 of falls cause injuries that limit the older adults’ activities for at least a day or need medical attention. Approximately 3 million emergency room visits and one million hospitalizations among older persons are due to falls (CDC, 2024). FRM is crucial in preserving patient mobility and functional independence, improving quality of life, and reducing fall-associated healthcare burdens (Dykes et al., 2020; Montero-Odasso et al., 2021).

Clinical practitioners should implement regular fall risk assessment/ screening and develop individualized/ tailored interventions informed by the patient’s specific risk factors and needs (Dykes et al., 2020). Practical FRM interventions include but are not limited to strength, balance, and gait assessments/exercise, medication review for side effects that increase fall risk, vitamin D supplementation, and patient education on home safety measures (e.g. removing tripping hazards and installing handrails), and advocacy for safe hospital environment (e.g. anti-slip floors, perfect lighting) (Montero-Odasso et al., 2021).
