Purpose

The purpose of this discussion is for the DNP student to demonstrate an understanding of how stakeholders influence practice change. You will continually gain experience considering the organization, community, and national stakeholders and how they influence sustained practice change.

Instructions

  1. Identify and examine organization and/or community stakeholders you anticipate will be a part of the interprofessional team when implementing your future practice change project.
  2. Integrate your key stakeholders into the constructs from the translational science model that you identified in Week 6.
  3. Analyze the barriers you might experience with these stakeholders with the proposed practice change.

Solution: NR715 Week 7 Discussion Stakeholders

The practice problem identified is high rates of heart failure (HF), a leading cause of disability and death. The evidence-based practice change proposed is the implementation of a community-integrated patient education program for HF, combined with risk management strategies. The successful implementation of this change project requires interprofessional collaboration among different organizational and community stakeholders and the use of an appropriate translation model, the Knowledge-to-Action (KTA) Framework. Among the stakeholder groups to be integrated into the KTA model for implementation of the change project are healthcare providers including primary care physicians (PCPs) who assess and diagnose HF risk, provide personalized education, and guide clinical management; nurses and nurse educators who educate patients on HF management during clinic visits and workshops; and cardiologists who offer expertise on complex HF cases and advice on clinical risk management strategies (Archana et al., 2020). Other stakeholders include community health workers (CHWs), public health officials, local government and nonprofit organizations, patients and caregivers, information technology (IT) teams, pharmacists, dietitians, and exercise specialists (Abu‐Odah et al., 2022).
