John is a 58-year-old male who presented with complaints of worsening shortness of breath over the past few months. He reported feeling easily winded with mild activity such as walking up stairs. He also stated his ankles had been swelling more at the end of the day. John had a medical history significant for hypertension and was pre-diabetic. His blood pressure had been difficult to control over the years despite being on multiple medications. Upon examination, John’s blood pressure was elevated at 150/90. He also had pitting edema in his lower extremities. Based on his symptoms and risk factors, there was consideration for a possible heart failure.
